New Clients

Welcome to Westoak Animal Hospital

We are happy to welcome new clients and patients to our full-service practice and look forward to working together to help your pets live longer, healthier lives. 

To help you prepare for your first visit with us, we have provided some information below. 

1.  NEW CLIENT FORM

  • Prior to your appointment please complete the "New Client Intake Form".

  • Two options to complete the form:

     

(i)  Type directly on the editable Word document (Tip: TAB through the document) or

(ii) Print the PDF and hand write in the details.

2.  WHAT TO BRING

  • Completed "New Client Intake Form"

  • Previous vaccine and medical history

  • Fecal sample for puppies, kittens or if your pet has gastrointestinal issues (ie diarrhea)

3.  WHAT TO EXPECT

When you arrive, you will be warmly greeted by one of our Receptionists. Our Receptionist will then gather your documentation (ie patient history), weigh your pet and place you in one of our exam rooms, once one is available. 

In most cases, the first person to enter the room is one of our Veterinary Technicians who will take a  history and gather other information for Dr. Kohlmaier or Dr. Fisher. 

Afterwards, one of our Veterinarians will come in. They will perform a physical examination, gather additional information, answer questions or address your concerns, as well as provide their recommendations.

At the end of your visit, our Receptionist will handle the billing, ensure you have received any medications prescribed (if applicable) and book recheck/follow-up appointments, if required.
